In-Center Hemodialysis Licensed Practical NurseSupports the mission, core values, and customer service of Dialysis Care Center, LLC (DCC).Adheres to the DCC Policies and Procedures and Compliance Program, including all state and federal regulatory requirements.The In-Center Hemodialysis Licensed Practical Nurse provides safe, high-quality nursing care under the direction of the Registered Nurse and Clinical Nurse Manager while supporting operational excellence, regulatory compliance, patient safety, and exceptional patient outcomesPrimary Duties / Activities:Patient Related:Provide direct patient care for assigned patient(s).Perform all technical aspects of dialysis procedures for assigned patients as prescribed. Administer and monitor treatments in accordance with current treatment plan or as directed by the supervisor.Monitor and document dialysis treatment parameters on dialysis flow sheets. Document other information related to the care of the patient in the individual patient record.Responsible for reporting any significant information, change in patient condition, or equipment problems to the supervisor.Completes assigned Daily Opening and Closing ProceduresMonitors DCC Quality Metrics: Kt/V, Anemia, Catheter Rate, Access Infections, Missed treatments, etc.Participates in DCC Missed Treatment Intervention initiatives by assisting with patient outreach, rescheduling, and interdisciplinary communication regarding missed or shortened treatments.Assist in developing and following the teaching plan to educate the patient and family regarding end stage renal disease, dialysis therapy, diet and medications.Documents patient care accurately and timely in the Electronic Medical Record.Reviews documentation for completeness before treatment closure.Assist with all emergency operational procedures.Administer intravenous medication in accordance with physician orders and state nurse practice laws.Communicates all emergencies, patient life-threatening complications, and related situations to the attention of the medical director, physician, and Clinic Manager. Maintains competency with all emergency operational procedures. Ensures verification and availability of adequate emergency equipment.Assess patients' response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as prescribed by physician, assessing target weights.Ensures compliance with safety and infection preventative measures, performing safety and infection audits on a routine basis.Ensuring patient safety and monitoring adverse Event Occurrences.Perform and document pre, interim, and post treatment review of patient condition.Initiate basic CPR measures in the event of cardiac and/or pulmonary arrest and respond to emergency situations related to dialysis treatment.Technical:Safely operate all dialysis related equipment according to the proper procedures.Provide minor troubleshooting when necessary.Ensure a clean, safe and sanitary environment in the dialysis facility treatment area.Operate all emergency equipment effectively in accordance to the standard operation procedures.Staff Related:Assist in short term and long-term patient care plan meetings.Utilizes DCC Labor Logic to optimize staffing, labor productivity, scheduling efficiency, overtime management, and patient treatment scheduling while maintaining safe staffing standardsProvides clinical direction to Patient Care Technicians within scope of practice under RN/CNM supervision.Successfully completes all DCC University training modules.Participates in mandatory continuing educationCompletes Staff Nurse Water Quality Facility Training.Train new and current staff as needed. Assists with the orientation of new employees. Assists with competency validation.Act as a resource person for other staff members. Assist with the coordination of the care plan with Physician, Dietitian and Social Worker.Reinforces Home Hemodialysis and Peritoneal Dialysis education and notifies the Registered Nurse when patients express interest.Maintains continuous survey readiness.Participates in mock CMS and state surveys.Float between facilities as needed and work a flexible schedule (including mornings, evenings, weekends, and holidays)Staff scheduled hours are based on the needs of the facility and are subject to change.Ensures mandatory in-services and annual competencies are completed as required.Participates in all required staff meetings.Travel to other locations as needed.Other duties, as assigned.Customer Service:Responsible for supporting the values and customer service standards of DCC.Accountable for outstanding customer service to all staff, clients and vendorsDevelops and maintains effective relationships through effective and timely communication.Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.Education/Qualifications:Graduate of an accredited School of Practical Nursing (LPN, LVN)Current state licensure as a Licensed Practical Nurse or Licensed Vocational NurseMaintains state licensure in good standing.Current CPR CertificationExperience:Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a mustGood presentation & communication skillsMust be productive & resourceful with the ability to meet deadlinesA minimum of 6 months hemodialysis experience preferred.Physical Demands and Working Conditions:The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. May be exposed to infectious and contagious diseases/materials. The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may assist. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force.
