Full-time work is typically conducted Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as client needs demand. The role necessitates adaptability, as you may be required to be on-call at times to address urgent situations effectively and ensure continuity of care for our clients. Key Responsibilities\n \n Conduct comprehensive psychosocial assessments of seniors facing health challenges, ensuring a thorough understanding of their emotional, mental, and social needs. \n Develop and implement individualized care plans in partnership with clients and their families, aligning with their preferences, goals, and available resources. \n Provide empathetic counseling and emotional support to clients and their families to help them navigate complex life transitions and challenges associated with palliative care. \n Act as an advocate for clients, helping them gain access to vital community resources, services, and support needed for their well-being. \n Collaborate with interdisciplinary teams, including nurses, healthcare providers, and caregivers, to coordinate comprehensive care and enhance client outcomes. \n Conduct regular check-ins and visits to monitor clients' conditions, address any concerns regarding their living situations, and ensure safety. \n Facilitate open communication among family members involved in the care process, helping resolve conflicts and fostering supportive relationships. \n Educate clients and families about available community support services, social engagement programs, and resources to foster well-being. \n Maintain accurate and timely documentation of client interactions, assessments, care plans, and progress notes in accordance with organizational standards. \n Stay updated with the latest practices and policies in social work to ensure compliance with ethical standards and proficiency in service delivery. \n Patient Population / Client Focus\n This role focuses on serving senior clients within the hospice care setting, many of whom may be dealing with terminal illnesses and significant life changes. The challenges they face often require a nuanced understanding of both emotional and social support systems. Your role will be pivotal in enabling clients and their families to cope with these challenging experiences, enhancing their quality of life during a sensitive period. Qualifications\n \n A Master’s degree in Social Work from an accredited institution. \n Current and valid licensure as a Licensed Master Social Worker (LMSW) in Illinois. \n A minimum of two years of professional experience in social work, preferably within hospice or geriatric care settings. \n A thorough understanding of social work principles, community resources, and aging-related issues. \n Exceptional assessment, counseling, and problem-solving capabilities. \n Empathetic, compassionate approach to interacting with seniors and their families. \n Strong communication and collaboration skills for effective teamwork with healthcare professionals. \n Proficiency in utilizing electronic health record (EHR) systems and relevant computer applications. \n Preferred Experience\n \n Experience specifically in hospice care or related fields, working with end-of-life issues. \n Familiarity with local community resources available for senior clients and their families. \n Knowledge of therapeutic interventions specific to older adults, including grief counseling. \n Compensation and Benefits\n The compensation for this role ranges from $60,000 to $100,000 annually, based on experience and qualifications.
