Shifts: Monday – Friday 8am-5pm, on call every 3rd weekend and 1-2 nights during the week
Seven Healthcare are seeking an experienced RN specializing in Case Management for a contract-to-hire role in Glasgow, KY. This travel assignment requires a skilled nurse to perform comprehensive assessments and collaborate with primary care providers, ensuring high-risk members receive the care they need. Key Responsibilities of the RN – Case Management
- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers for individualized care planning.
- Address and reassess cost-efficient levels of care.
- Implement non-medical support to ensure treatment compliance.
- Coordinate care transitions as needed.
- Work with physicians and care teams for patient care transitioning.
- Engage specialty resources for optimal resolution.
- Keep detailed records of outcomes during management.
- Identify opportunities for health promotion and illness prevention.
- Act as a patient advocate by protecting privacy and confidentiality.
Requirements of the RN – Case Management
- Associates or Bachelor's Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Ability to analyze complex medical information and make rational decisions.
- Case Management certification.
- KY RN- compact license.
