Job Description

Essential Duties: Responsibilities

• Adheres to and conveys a philosophy that supports the dignity, privacy, independence, choice, and individuality of patients.
• Transports patients according to schedule, staying within allotted times.
• Assists patients in and out of the vehicle.
• Interacts with patients in a warm, professional manner, maintaining positive relationships with patients.
• Notifies the Administrative Assistant of any potential mechanical problems or needed repairs.
• Maintains an open line of communication with the Administrative Assistant regarding needed transportation.
• Maintains a clean driving record.
• Responsible for insuring that all required emergency equipment (i.e., triangles, fire extinguisher, and first aid kit) are available and in good working order.
• Keep vehicle logs accurately and completely.
• Insure that the vehicles gas, oil and other fluids are kept at appropriate levels.
• Other responsibilities/accountabilities: Duties and responsibilities may be added, deleted or changed to meet the facility's needs, at the discretion of the Administrative

Qualifications

Qualifications:

Basic Knowledge/Skills:

• Good verbal and written communication skills
• High school diploma or equivalency
• Computer skills: MS Office Suite
• Able to work with minimal supervision successfully

• Bilingual (Spanish/English) preferred
• CPR Certification

Experience and Education:

• High school diploma or GED
• Must have a current Arizona driver's license
• Must have been licensed for 3 years
• Must be a minimum of 21 years old
• Must meet Circle the City insurer's requirements for safe past driving record.
