Position Overview

BrightSpring Health Services is seeking a Physical Therapist for a Home Health assignment in Beckley, WV. This position offers the opportunity to work independently while providing skilled therapy services in the patient's home environment. The PT will evaluate patients, establish treatment plans, coordinate care with the interdisciplinary team, and help patients maximize safety, mobility, and independence.

Responsibilities

• Complete evaluations and reassessments
• Develop and implement individualized treatment plans
• Provide skilled physical therapy interventions in the home
• Collaborate with physicians, nurses, caregivers, and therapy staff
• Complete timely and accurate documentation
• Educate patients and families on safety, mobility, and home exercise programs
• Participate in care planning and discharge planning

Requirements

• Active West Virginia Physical Therapist License
• Current BLS Certification
• Home Health experience preferred
• OASIS experience preferred
• Valid driver's license and reliable transportation
• Strong communication and time-management skills
