BAS Healthcare is partnering with a mission-driven, community-focused healthcare organization to recruit a Board-Certified or Board-Eligible Internal or Family Medicine Physician for a full-time outpatient opportunity in East St. Louis, Illinois.   100% Outpatient Internal/Family Medicine

• \n

  Provide comprehensive care for adult patients, managing acute and chronic conditions

\n
• \n

  Perform physical exams, diagnostic evaluations, and develop treatment plans. 15-minute follow ups, 30-minute initial visit, can use more time if needed.

\n
• \n

  Prescribe medications, administer immunizations, and manage chronic disease care

\n
• \n

  Emphasis on preventive care , wellness visits, and population health

\n
• \n

  Deliver culturally sensitive care with attention to social determinants of health

\n
• \n

  Collaborate closely with a multidisciplinary care team, including APPs, nurses, and specialists

\n
• \n

  Participate in community outreach and health education initiatives

\n
• \n

  Document all encounters accurately and timely in the EHR

\n
• \n

  Monday–Friday daytime schedule

\n
• \n

  Occasional travel to other clinic locations, as needed

\n

\n
• \n

  MD or DO

\n
• \n

  Board Certified or Board Eligible in Internal Medicine

\n
• \n

  Eligible for or licensed to practice in the state of Illinois

\n
• \n

  Strong commitment to patient-centered, community-based care

\n
• \n

  Comfort working in a fast-paced, team-oriented outpatient environment

\n
• \n

  Experience serving diverse or underserved populations preferred
