Oasis Therapy Services is seeking a Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation services at an assisted living facility in Cannon Falls, Minnesota.

Responsibilities
- Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
- Carry out individualized intervention plans developed by the supervising Occupational Therapist, focusing on ADLs, mobility, safety, and independence.
- Monitor residents' progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
- Educate residents and caregivers on exercises, adaptive techniques, and equipment.
- Complete accurate Net Health documentation and collaborate with the care team.
Qualifications
- Active and current Minnesota Occupational Therapy Assistant license.
- Active NBCOT COTA certification in good standing.
- Graduate of an accredited Occupational Therapy Assistant program.
- Current CPR certification.
- Strong communication, organization, and documentation skills.
- Ability to work independently while following required Occupational Therapist supervision.
- Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred.
