RequirementsBoard Certification

• Board Certified Family Medicine
• Board Certified Internal Medicine

License

• Active Arizona License
• IMLC Accepted (Letter of Qualification must be in hand at time of submission)

DEA

• Active DEA Highly Preferred

Experience

• Minimum 2 years of Family Medicine or Internal Medicine experience
• Geriatric experience preferred
• Primary Care physicians with geriatric experience will be considered

Practice Details

• Outpatient Clinic Setting
• No Trauma
• EMR: Cerner
• Support Staff: Medical Assistant
• Reason for Coverage: Departing Provider

ScheduleClinic Hours

• Tuesday - Friday
• 7:00 AM - 5:30 PM
• 30-minute unpaid lunch

Call Coverage

• Sporadic night call
  • 5:30 PM - 7:00 AM
• Sporadic weekend call
  • 7:00 AM - 7:00 AM

Patient Population

• Adult
• Geriatric
• Majority Geriatric Practice

Clinical Responsibilities

• Annual Wellness Visits (AWVs)
• Acute Care Visits
• General Primary Care
• Geriatric Medicine Management

Volume

• Approximately 18 patients per day

Ideal Candidate

• Board Certified FM or IM physician
• Experience caring for geriatric populations
• Active Arizona license or IMLC LOQ in hand
• Able to provide specific coverage dates between August and December 2026
• Comfortable with outpatient clinic and occasional call coverage
