Compassionate Hospice RN Case Manager | Day Shift
Heartland of Perrysburg
Heartland Hospice in Akron, OH is seeking an RN Case Manager for a full-time dayshift, Monday through Friday. You will be the primary contact for patients and families, delivering and documenting skilled nursing care in the patient’s home or other settings, and coordinating with an interdisciplinary team to implement individualized plans of care.
The role emphasizes compassionate, patient-centered care, education for caregivers, and thorough clinical documentation while maintaining compliance
