Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum 2 years of acute care hospital case management experience License: California state license required; pending license accepted but must be licensed upon start date Certifications: Not specified Must-Have: - 2 years of experience in acute care hospital case management, including care coordination and discharge planning - Knowledge of assessing patients, care coordination, and discharge planning - Familiarity with drug/alcohol abuse treatment options and mental health issues - Experience in organizing and participating in family meetings - Competency in arranging durable medical equipment and transport - Proficiency in computerized documentation and Microsoft applications Description: This position requires performing care coordination and discharge planning in an acute care setting. The nurse will be responsible for managing a patient ratio of 1:20. Candidates must have prior hospital experience, and experience with first-time travelers is accepted. The role includes weekend rotation and supporting departmental needs. Proficiency with required charting systems is essential. Knowledge of advance directives and the protective services process is necessary. Applicants must be prepared to float between different facilities within the state as required by the position. The role requires participation in treatment plan development for patients across various demographics. Travelers must expect to comply with department policies and procedures.
