Community Health Worker - Hybrid

Job Description

A growing healthcare technology company is seeking a Community Health Worker to support patient engagement, care coordination, and enrollment efforts focused on improving cardiovascular care.

Role Highlights

Hybrid position: 2–3 days remote / 2–3 days in the community
Travel to hospitals and healthcare facilities throughout Chicagoland
Engage patients in person, by phone, text, and email
Educate patients about available care programs and support enrollment
Build relationships with patients, caregivers, hospital teams, and clinical staff
Help coordinate discharge plans and successful transitions back home
Maintain patient documentation and activity within the company's EHR
Collaborate with an interdisciplinary healthcare team
Meet monthly outreach, enrollment, and engagement goals

Ideal Background

3+ years of healthcare experience in a hospital, clinic, physician practice, or similar environment
Background as a Community Health Worker, Medical Assistant, LPN, CNA, or similar role
Cardiology experience preferred
Strong patient communication and relationship-building skills
Comfortable explaining healthcare programs and engaging patients
Tech-savvy and able to learn new systems quickly
Care coordination or interdisciplinary team experience preferred
Telehealth experience is a plus
Spanish-speaking is a plus
Valid driver's license, reliable transportation, and ability to travel locally
Available Monday–Friday, 8:00 AM–5:00 PM
