Key Responsibilities\n- \n
- Conduct thorough psychosocial assessments to evaluate emotional, social, and environmental factors that affect each patient's recovery. \n

- Work closely with the rehabilitation team to create individualized treatment plans that incorporate the patients' psychosocial needs and goals. \n
- Assist patients in navigating community resources, linking them to support groups, vocational rehabilitation services, and housing support as needed. \n
- Educate patients and families on post-rehabilitation options, ensuring a smooth transition back to home or other care settings. \n
- Participate actively in discharge planning meetings, contributing insights on patients' readiness for transition and ongoing care needs. \n
This role entails working with a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will support these individuals and their families through challenging emotional and social transitions, fostering resilience and reinforcing coping strategies crucial for their rehabilitation and reintegration into their communities.

- A Master's degree in Social Work from an accredited institution is required. \n
- Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory. \n
