General Summary/Primary Purpose

The Aftercare case manager is responsible for assessment of and service delivery to participants in the TRAILS program and Sullivan House facility. The Aftercare case manager facilitates groups in the 90-day, Aftercare and possibly Sullivan House residents. This position also provides individual case management for residents of Sullivan house who are not part of the TRAILS program. The case manager is responsible for ongoing support, follow-up, and strict supervision according to program/house procedures.

Examples of Duties

Develop and maintains professional relationships with community treatment providers and community employers. Informs a potential employer about benefits of employing inmates: Work Opportunity Tax Credit.

Facilitates education sessions/classes/groups as directed by the Program Director, to include job readiness and life skills.

Maintains tracking data in Excel spreadsheets. Works with protected information including HIPAA and 42 CFR protected data.

Develops, monitors, and maintains individual service plans for aftercare participants and Sullivan House individuals by making appropriate referrals for medication management, housing, clothing, food, and employment.

Acts as a liaison to community members and organizations representing the facility in a professional manner

Keeps the Program Director informed of inmate progress throughout the person's incarceration

Establish an inventory of available housing options for released inmates.

Collaborates and meets with Clinical and Sullivan house teams on a weekly basis to ensure that inmate goals are being met

Maintain records and notes according to program procedures and facility policies. Completes all required paperwork in a timely manner to assure a smooth transition through the facility from intake to release.

Any other duties deemed necessary by the Program Director

Typical Qualifications

The candidate for this position shall possess a Bachelor's Degree in education, rehabilitation counseling or human services from an accredited college or university.

The successful candidate will have a thorough knowledge of the principles of correctional institution operations, knowledge of criminal codes, knowledge of facility rules and regulations, skills training and instruction, have the ability to use computers, manage difficult people under stress and is able to communicate effectively.

The candidate must possess a current New Hampshire driver's license and pass a physical examination and drug screen. The candidate must be capable of social and professional interaction on varying levels of cognitive behaviors to include educational, literacy, cultural, developmental disabilities, health and biomedical, as well as community needs such as housing and dependent family situations.

Supplemental Information

Work is performed both at the Department of Corrections in Unity and at the Sullivan House in Claremont. While some of the hours may provide flexibility, the candidate would be expected to work until 7pm on Tuesdays, weekend days are an option. The candidate works in a moderately noisy setting and is exposed to outdoor weather conditions for limited periods. There is a regular risk of personal injury when violent/hostile situations require crisis intervention and emergency response; there is the potential for exposure to infectious diseases, blood borne pathogens and mental fatigue.

Physical demands generally involve sitting, stooping, climbing stairs, standing reaching with arms/hands, walking, talking, listening or hearing and using hands more than 2/3 of the time. Occasionally weigh is lifted or force exerted up to 30lbs, seldom is weight lifted or force exerted more than 30 lbs. Close vision requirements are necessary in order to detect contraband or avoid injury. Equipment used includes automobiles, office machines, and computers.

Duties at this level are predictable and are referenced by laws and court decisions; however, diligence must be used to resolve abnormal situations. Errors made at this level could result in personal injury, loss of service and damage to building or equipment as well as monetary loss.

The candidate has frequent contact with the public through meetings with inmate's families, community groups, volunteers and through other programs to facilitate an ongoing, progressive relation with participants. Other contacts are typically courts, sheriff's department, police departments, probation, social services and counseling authorities.
