Physical Therapist

We are seeking licensed Physical Therapists to provide personalized, in-home and community-based care to older adults in various locations across the Greater Boston area. Key Responsibilities Include Performing evaluations, creating individualized treatment plans, and delivering evidence-based therapy to support mobility, independence, and overall well-being. Providing one-on-one care in private residences and senior living communities, and utilizing telehealth when appropriate. Documenting patient progress accurately within electronic medical records and collaborating with interdisciplinary teams. Educating patients, caregivers, and staff to promote safety and aging in place. Qualifications And Skills Required Valid Physical Therapy license or eligibility in the state. Degree from an accredited program and proficiency with EMR systems. Current CPR certification. Ability to meet physical demands, including lifting, repositioning, and assisting patients, along with driving to patient locations.
