Job Summary:

Seeking an experienced Physical Therapist to provide skilled therapy services in a nursing home setting. This position offers an opportunity to work part-time on Saturdays, contributing to patient recovery and well-being in a skilled nursing facility.

Responsibilities:

• 
  Evaluate patient mobility, strength, and functional ability to develop personalized treatment plans.
  
  
• 
  Deliver evidence-based physical therapy interventions to improve patient outcomes.
  
  
• 
  Monitor patient progress and adjust therapy plans accordingly.
  
  
• 
  Document patient assessments, treatment sessions, and progress notes accurately.
  
  
• 
  Collaborate with nursing staff and other healthcare professionals to coordinate patient care.
  
  
• 
  Adhere to all licensing, safety, and documentation standards in compliance with regulatory requirements.
  
  

Qualifications:

• 
  Valid NY Physical Therapist license required.
  
  
• 
  Prior experience working in a Skilled Nursing Facility is mandated.
  
  
• 
  BLS certification is required.
  
  
• 
  Strong assessment skills and ability to create tailored treatment plans.
  
  
• 
  Excellent interpersonal and communication skills.
  
  
• 
  Ability to work independently and as part of a multidisciplinary team.
