Position Summary:

We are currently seeking a PRN Physical Therapist (PT) to join our dedicated team. In this flexible, as-needed role, you will evaluate patients and implement individualized physical therapy treatment plans aimed at restoring function, relieving pain, and preventing disability due to injury, illness, or disease. You’ll work collaboratively with other therapy professionals and nursing staff to ensure residents receive the highest standard of rehabilitative care.

Responsibilities:

• Assess residents and develop customized treatment plans in accordance with physician orders.

• Deliver physical therapy services that align with clinical standards and best practices.

• Document patient progress accurately and promptly in medical records.

• Supervise and guide Physical Therapy Assistants (PTAs) and support staff.

• Educate residents and caregivers on continued care strategies and exercises.

• Maintain compliance with federal, state, and facility regulations.

   

Qualifications:

• Licensed Physical Therapist in the state of Georgia.

• Graduate of an accredited PT program.

• CPR certification (preferred).

• Previous experience in a skilled nursing or long-term care setting strongly preferred.

• Strong communication, organizational, and interpersonal skills.
