Position Summary:The Registered Dietitian Nutritionist (RDN) provides nutritional assessments, Medical Nutrition Therapy (MNT), and nutrition education for PanCare patients who have, or are at risk for, diet-related chronic conditions. The position supports PanCare's HRSA Telehealth Nutrition Services Network Grant Program (NS-TNGP), Grant and works with patients one-on-one and in groups, by telehealth and in person. The RDN serves as a distant-site provider within PanCare's telehealth network, delivering nutrition services by telehealth, including remote patient monitoring, to patients at network originating sites located in rural and medically underserved areas. works alongside a dedicated Community Health Worker, who handles outreach, enrollment, and care coordination. Together they connect nutrition care to primary care, behavioral health, and prenatal services. The position is home-based and makes site visits as needed. It also leads food-based interventions, EHR integration, UDS reporting accuracy, and quality improvement for nutrition services.Responsibilities:• Collaborates with originating-site clinical staff and network partners to receive referrals, coordinate patient care, schedule telehealth visits, and ensure that nutrition recommendations are communicated to each patient's primary or specialty care team.• Conducts comprehensive nutritional assessments for patients referred by primary care. Conditions include diabetes, hypertension, hyperlipidemia, cardiovascular and ischemic vascular disease, overweight and obesity.• Provides MNT and individualized nutrition counseling based on evidence-based practice guidelines and each patient's treatment plan, within the scope of Florida dietetics and nutrition licensure.• Delivers individual visits by telehealth using PanCare's telehealth platform and in person at assigned service sites as required. This helps reach patients who face transportation, distance, or scheduling barriers.• Uses remote patient monitoring (RPM) and other telehealth technology to assess, evaluate, and track patient progress between visits, as described in the approved NS-TNGP project plan.• Supports the onboarding of originating sites to the network, including coordinating start dates for services, orienting site staff to the referral and telehealth workflow, and providing the clinical and service information needed for MOAs.• Maintains regular communication with network partner sites, including recurring check-ins to address workflow, referral volume, and service delivery issues, and participates in network meetings.• Reviews RPM data on a defined schedule, identifies concerning trends or missed readings, adjusts nutrition care plans accordingly, and contacts patients or notifies the referring provider and care team when results fall outside established parameters.• Works with the Community Health Worker and originating-site staff to enroll eligible patients in RPM, provide device setup and use instruction, and support patients who have difficulty with technology or connectivity.• Documents RPM-related nutrition services and patient data in the EHR in accordance with PanCare's clinical documentation, privacy, and security policies, and with applicable billing requirements.• Develops, documents, and reassesses individualized nutrition care plans. Coordinates with referring providers and the care team and takes part in huddles and case conferences for patients with complex or poorly controlled conditions. • Keeps a visit schedule that meets the program's patient and visit commitments, including timely initial assessments and structured follow-up.• Plans and leads recurring group nutrition visits and classes, including weight management programming and condition-specific series such as diabetes nutrition education.• Leads cooking demonstrations and meal-preparation instruction. Develops food literacy curricula on selecting, storing, preparing, and budgeting for nutritious food, adapted for pediatric patients and caregivers, prenatal patients, and migratory and seasonal agricultural workers and their families.• Develops and maintains patient education materials written for the health literacy levels of the populations served.• Designs and oversees food-based interventions, including produce prescriptions filled through contracted local farms or grocers. Partners with the Community Health Worker to connect food-insecure patients with community food resources.• Makes sure food handling, storage, and distribution activities follow federal, state, and local food safety requirements and PanCare's food distribution policies.• Builds and maintains referral pathways into nutrition services from primary care, behavioral health, and prenatal care. Works with clinical informatics on EHR referral orders, prompts, order sets, care plan templates, and clinical decision support.• Documents all nutrition services in the EHR using the discrete, reportable fields needed for Uniform Data System (UDS) reporting. This includes the nutrition services measures in UDS Table 5 (Staffing and Utilization) and Table 6A (Selected Diagnoses and Services Rendered). Documents site-level service delivery (patients served, visits completed, and encounter type) to support network and HRSA reporting, and maintains records showing that services are delivered to originating sites located in rural and medically underserved areas.• Implements and monitors nutritional status and food insecurity screening, including selecting and rolling out a validated screening tool.• Takes an active part in PanCare's QI/QA program. Reviews nutrition service performance data, adjusts program delivery based on results, and supports AAAHC survey readiness.• Follows and abides by facility policies, procedures, and Corporate Compliance Program, as applicable to role and responsibilities. Maintains required training. • Demonstrates commitment to organizational compliance and quality standards by following established policies and procedures, maintaining regulatory requirements, participating in quality improvement efforts, and reporting concerns through appropriate channels.• Perform other duties as assigned to support business operations.Required Skills/Abilities:• Ability to build working relationships with partner sites and to deliver care within a multi-site telehealth network.• Ability to learn and use RPM platforms and telehealth technology, and to interpret patient-generated health data to guide nutrition care.• Thorough knowledge of MNT and evidence-based nutrition guidelines for chronic disease, pediatric, and prenatal populations.• Ability to use independent professional judgment in assessing patients, developing care plans, and designing programs.• Strong counseling and group facilitation skills.• Ability to communicate in plain language with patients of diverse cultures, ages, and health literacy levels.• Proficiency with EHR documentation and Microsoft Office Suite.• Ability to work independently in a home-based setting while staying connected to care teams.• Strong organizational skills and the ability to manage a caseload against program deliverables.• Commitment to patient confidentiality and HIPAA.• Regular and predictable attendance is required to support business operations.Education/Experience:• Bachelor's degree or higher in dietetics, nutrition, or a closely related field from an accredited institution, plus completion of an ACEND-accredited supervised practice program. Required.• Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration. Required.• Current Florida Dietitian/Nutritionist license (LD/N), or eligibility to be licensed before the start date. Required.• Demonstrated experience providing MNT and nutrition counseling to patients with diet-related chronic conditions. Required.• Ability to pass background screening and meet all credentialing and privileging requirements applicable to the position. Required.• Bilingual in English and Spanish. Preferred.• Experience in an FQHC, community health center, or other safety-net setting. Preferred.• Experience with group nutrition education, cooking instruction, or teaching kitchen programs. Preferred.• Experience providing nutrition services by telehealth. Preferred.• Experience using RPM or digital health tools in nutrition or chronic disease care.
