Title : RN - Home Health
Duration : 3 months contract + Possibility of Extension
Location : WESTOVER, WV
Coverage Areas: Ohio, Marshall, Wetzel and Tyler Counties
Shift : M-F 8am-4:30pm
On Call/Weekend Requirement : at least one weekend a month along with a Friday and 4 other weekdays.
Pay Rate: $49/hr - $50/hr

Roles & Responsibilities:
Conduct skilled nursing assessments in patients' homes
Complete OASIS visits, including:
Start of Care (SOC)
Resumption of Care (ROC)
Recertifications
Follow-up assessments
Discharge assessments
Evaluate patient conditions and identify changes that require intervention or care plan modifications
Deliver care in accordance with physician orders and individualized care plans
Monitor patient progress and clinical outcomes
Perform medication reconciliation and patient education
Support disease management and promote safe care in the home environment
Utilize Homecare Homebase (HCHB) for patient documentation and care coordination

Minimum Qualifications:
Active and unrestricted Registered Nurse (RN) License
Experience with OASIS assessments and documentation
Experience using Homecare Homebase (HCHB)
Valid driver's license
Reliable transportation and current automobile insurance

Preferred Qualifications:
Previous Home Health Nursing experience
Current and up-to-date clinical skills across a variety of patient populations
