Job Title Clinical Team Manager (RN) – Leadership Development Opportunity Responsibilities Leading and supporting a team of field clinicians. Coordinating and overseeing patient care plans. Coaching and developing nurses and clinical staff. Ensuring quality care and compliance with clinical standards. Partnering with leadership to support continued growth in the community. Qualifications Current Registered Nurse (RN) license. Home health and clinical experience. Experience with home health clinical documentation, including OASIS and electronic medical record systems such as Homecare Homebase. Interest in developing leadership and management skills. Strong communication and collaboration abilities. Reliable transportation and local travel. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
