TÉRMINOS DE REFERENCIA - SERVICIO DE MÉDICO OCUPACIONAL
1. ANTECEDENTES
NESsT es una organización internacional sin fines de lucro que impulsa el desarrollo de empresas sociales con el objetivo de generar impacto social y ambiental sostenible. En el marco de su Programa de Seguridad y Salud en el Trabajo (SST) y considerando que cuenta con 15 colaboradores en la oficina de Perú, NESsT requiere contratar el servicio de un médico ocupacional para cumplir con las obligaciones vinculadas a la vigilancia de la salud de sus trabajadores.
2. OBJETO DE LA CONTRATACIÓN
Contratar el servicio de un Médico Ocupacional para la lectura, interpretación y análisis de resultados de exámenes médicos ocupacionales, así como la elaboración del Informe Técnico de Vigilancia De la Salud De Los Trabajadores (ITV) y su presentación ante DIGESA, en cumplimiento de la Ley N°29783, DS 005-2012-TR, RM 312-2011/MINSA y normativa DIGESA vigente.
3. ALCANCE DEL SERVICIO
Incluye lectura de resultados, análisis epidemiológico, elaboración del Informe Técnico de Vigilancia y presentación ante DIGESA.
4. PRODUCTOS ENTREGABLES
- Informe de revisión y análisis de EMO s.
- Informe ITV en formato Excel DIGESA.
- Carta de presentación del ITV firmada.
- Informe ejecutivo y constancia de presentación ante la entidad indicando el Nro. De expediente.
- Informe final de la consultoría.
5. PERFIL DEL PROVEEDOR
Médico ocupacional colegiado, con experiencia comprobada en vigilancia de la salud.
6. PLAZO DEL SERVICIO
El servicio deberá ejecutarse en un plazo máximo de hasta 1 5 días calendario, contados desde la entrega completa de la información por parte de la empresa.
- - Lectura y validación de resultados: 5 a 7 días calendario
- - Análisis y elaboración del Informe Técnico de Vigilancia: 3 a 5 días calendario
- - Presentación ante DIGESA: 1 a 2 días calendario
El plazo podrá ajustarse en función de la cantidad de trabajadores evaluados y la calidad de la información proporcionada.
7. CONFIDENCIALIDAD
- Garantizar confidencialidad de datos médicos individuales.
- Reportar únicamente información agregada en el ITV (sin vulnerar datos personales).
8. FORMA DE PAGO
- Pago contra entrega y conformidad de:
- Informe ITV
- Constancia de presentación a DIGESA
9. RESPONSABILIDADES CLAVE
- El médico ocupacional es responsable de:
- La veracidad del análisis
- La correcta interpretación médica
- El cumplimiento del formato DIGESA
