Minimum Requirements

Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA

Active Arizona Physical Therapist license required to start the assignment

Current CPR/BLS certification from AHA or ARC may be required

Home Health experience required. Must be familiar with home visits and evaluating home environments

Experience supervising or collaborating with PTAs and Home Health Aides preferred

2-3 years of physical therapy experience preferred

Proficiency with basic computer functions, electronic documentation, email, and online training platforms

Reliable transportation, valid driver's license, and agency-required liability insurance for home visits
