Case Manager I

Location: Long Beach, CA, US Salary: $27.77 to $31.36 per hour Position Summary Seeking a full‑time Housing Case Manager to join our CalAIM – Community Supports team. The Housing Case Manager will offer case management services to Justice‑Involved (JI) individuals transitioning from homelessness to temporary or permanent housing or assist those formerly homeless in retaining housing. The ideal candidate will have a client‑centered approach and provide excellent customer service, addressing the challenges faced by homeless individuals with medical or behavioral health issues as they transition to permanent housing. The candidate will possess extensive knowledge of local resources and community agencies and demonstrate strong communication and problem‑solving skills to effectively address members’ needs. Experience conducting in‑home visits, visiting homeless encampments, and interacting within correctional facilities is essential. Preference will be given to candidates with personal experience of the justice system or a proven record of working with individuals involved with the justice system. Responsibilities Serve as the main contact and advocate for justice‑involved individuals. Conduct outreach, housing assessments, and create Housing Support Plans. Set goals, address barriers, and provide rental assistance. Link clients to health and support services, facilitate benefit establishment, and arrange transportation. Provide crisis management, eviction prevention, client and landlord education, and housing location services. Maintain active engagement and coordination of care with program‑enrolled members. Qualifications Minimum 1 year experience as a Housing Case Manager. Background working with individuals who have mental health conditions and substance use disorders. Flexible schedule that includes late evenings and weekends. Client‑centered approach and excellent customer service skills. Extensive knowledge of local resources and community agencies.
