Job Description Summary Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or an alternate infusion suite to ensure patient safety and continuity of care under a physician’s plan of care. The role operates under the structured supervision of a designated supervisor. Job Responsibilities Initiates, develops, and implements intermediate nursing plans of care, evaluating patient progress toward goals. Organizes and provides direct patient care, administers medications, and educates patients and families or caregivers. Modifies plans in response to changing patient status or physician orders to achieve established or revised goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re‑evaluates through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for treatment revisions and informs physicians promptly of significant changes in the patient’s condition, providing a written summary within supplemental orders. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Delivers effective, safe teaching using a patient‑centered approach for patients and families to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Communicates timely with Option Care’s Clinical team, medical providers, patients, and families or caregivers to facilitate continuity of care. Prepares clinical documentation in real time during visits and submits to the nursing department in accordance with billing timelines. Ensures documentation is legible and adheres to approved abbreviations and error‑correction practices per policy. Coordinates discharge planning, prepares discharge summaries with patient instructions, and reports patient care needs, progress, and goals when transferring care. Maintains compliance with agency operations, company policies and procedures, professional standards, local, state, federal regulations and accreditation standards. Maximizes work efficiency through computers and other automation technologies to validate treatment orders, communicate care provided, and follow assigned visit schedules. Responds promptly to patient requests and initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and procedures reflective of company mission, values, and objectives. Observes legal and ethical guidelines to safeguard confidentiality of patient and proprietary information, including HIPAA compliance. Communicates knowledgeably about the company’s scope of services and explains related financial obligations to patients. Attends required training modules, in‑services, and continuing education to maintain competency and licensure related to patient care in the home and alternate care settings. Provides oversight and coordination of paraprofessionals in the home setting and may act as supervisor in the absence of the supervisor or nurse manager as permitted by state and federal regulations. Participates in multidisciplinary team conferences, precepting, training, and mentoring of other nurses for orientation, onboarding, and supervisory activities as assigned. Participates in the nursing department on‑call responsibilities. Performs other related duties as directed by supervisor. If Applicable Insert PICC/Midlines per physician order. Troubleshoot/declort obstructed central line catheters. Monitor PICC/Midline and educate on prevention of line infection and routine care. Supervisory Responsibilities Does this position have supervisory responsibilities? No – X Yes Basic Education and/or Experience Active and unrestricted RN license in the state of practice. Minimum of 1 year of nursing experience. Current CPR certification required. Basic Qualifications Demonstrated competency in patient‑care standards required for safe delivery of services and infusion skill sets applicable to agency programs. Advanced certification and training as applicable. Valid driver’s license for the state of practice with access to a vehicle for business travel and proof of liability insurance. Basic knowledge of computer operating systems and software applications, including ability to evaluate annual performance and jointly set professional growth goals with the nurse manager. Physical Demand Requirements Ability to lift up to 50 pounds with a maximum lifting of 75 pounds; occasionally exert 20 pounds of force, frequently 10 pounds. Typical activities include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, and carrying. Requires fine motor skills and visual acuity for close and distant vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to blood, body fluids, and other potentially infectious materials; possible exposure to hazardous substances, needles, sharps, fumes, chemicals, humidity, temperature extremes, adverse weather, animals, and unpredictable home environments. Safety requirements include closed‑toe shoes, facemask, goggles, gown, gloves, and a functioning communication device for routine and emergency use. Travel Requirements Willing to travel up to 100% of the time for business purposes. Preferred Qualifications & Interests Bachelor of Science in Nursing from an accredited school preferred. 1–2 years of prior infusion nursing experience preferred. Care Provider Background Screening Employment is contingent upon successful completion of a company background check, drug screen, and enrollment in the Florida Care Provider Background Screening Clearinghouse.
