Hospice Registered Nurse (RN) – PRNCopper Sky HospicePhoenix, AZ

Copper Sky Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team on a PRN basis in the Phoenix area. This role is ideal for a nurse who is passionate about providing skilled, patient-centered end-of-life care while supporting patients and families with compassion, dignity, and professionalism.

As part of our interdisciplinary hospice team, you will play a key role in delivering high-quality care to patients in the comfort of their homes while helping ensure comfort, symptom management, and emotional support throughout the hospice journey.

ResponsibilitiesProvide Skilled Patient Care

• Deliver direct nursing care in accordance with physician orders and hospice plans of care.
• Administer medications, treatments, and interventions as appropriate.
• Perform comprehensive nursing assessments and monitor changes in patient condition.
• Ensure patients receive compassionate, individualized care focused on comfort and quality of life.

Care Coordination

• Serve as a case manager for assigned hospice patients.
• Collaborate closely with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team.
• Coordinate care plans and communicate patient updates to the care team and families.

Patient & Family Support

• Educate patients and caregivers on symptom management, medications, disease progression, and end-of-life care.
• Provide emotional support and guidance to patients and families during difficult transitions.
• Advocate for patient comfort, dignity, and patient-centered decision making.

Documentation & Compliance

• Complete accurate and timely clinical documentation.
• Maintain compliance with hospice regulations, agency policies, and Medicare guidelines.

QualificationsRequired

• Active Registered Nurse (RN) license in the state of Arizona.
• Minimum 1 year of professional nursing experience.
• Previous hospice experience required.
• Current BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, organization, and clinical assessment skills.

Preferred

• BSN preferred.
• Experience working with geriatric, chronically ill, or palliative care patients.
• Previous hospice case management or home health experience.
