A bit about us:

We are representing an established 40-bed inpatient rehabilitation hospital in Sioux Falls, South Dakota seeking an experienced Director of Case Management to lead and rebuild its case management function.

This is more than a maintenance leadership role. The incoming Director will have an opportunity to help create a stronger case management team, improve discharge processes, strengthen patient experience, and establish a positive culture within the department.

The hospital currently operates at an average daily census of approximately 40 patients, with all 40 beds configured as private rooms.

The organization strongly prefers an experienced RN case management leader with a BSN and meaningful hospital-based discharge planning experience.

Job Details

The Director of Case Management will lead hospital-based case management operations with particular emphasis on discharge planning, utilization review, patient experience, interdisciplinary coordination, and team development.

The department currently includes two full-time Case Managers and two contract Case Managers, with active searches underway for two additional full-time Case Managers.

Typical Case Manager caseloads are approximately 12-13 patients.

This role has significant opportunity for impact. Hospital leadership has been covering Director of Case Management responsibilities while the organization stabilized staffing, and the incoming Director will help establish the department's next chapter.

A major priority will be improving the patient experience surrounding discharge. The hospital has identified opportunities to improve both patient experience and discharge scores, making proven discharge-planning expertise especially important.

Responsibilities

Provide leadership and operational oversight for the hospital's Case Management department
Lead, coach, and develop full-time and contract Case Managers
Help recruit, onboard, and develop additional permanent Case Management staff
Oversee hospital-based discharge planning and utilization review activities
Improve discharge planning processes and address barriers affecting safe, timely transitions of care
Partner with physicians, nursing, therapy, social work, patients, families, and external providers
Drive improvements in patient experience and discharge-related performance
Establish a positive, accountable culture within the Case Management department
Maintain awareness of active patient cases and provide guidance on complex discharge situations
Support regulatory compliance, documentation standards, utilization management, and performance improvement initiatives

Qualifications

Minimum 3 years of hospital-based Case Management experience
Demonstrated experience with both Utilization Review and Discharge Planning
Candidates must be qualified to independently perform assessments within the scope of their professional discipline
RN candidates must possess a Bachelor of Science in Nursing
Active RN license required for nursing candidates
Social Work candidates must hold the appropriate professional licensure and possess meaningful medical or hospital-based experience
Bachelor's degree required for other eligible healthcare disciplines; graduate degree preferred
Proven ability to lead, influence, and develop Case Management professionals
Strong interdisciplinary communication skills
Strong knowledge of safe and effective discharge planning
CCM or ACM certification expected, or ability to obtain certification within the required timeframe
