Job Title: Registered Nurse (RN) Case Manager - Home Health Clinical Autonomy & Leadership Company: Home Health Care, Inc. Location: Greater Twin Cities Metro Area (21-county service area) Schedule: Full-Time, Monday to Friday, 8:00 AM - 5:00 PM, 1 weekend per month, 1 on-call shift per week Compensation: $85,000 - $130,000 annually plus $6,000 in first-year bonuses To stand out, email Tori at View email address on click.appcast.io with a summary of your RN experience, clinical strengths, and interest in home health . • Maintain accurate, compliant documentation . What We're Looking For
• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience, home health preferred.
• Strong clinical decision-making and assessment skills .
• Confidence working independently in a field-based role.
• Organized, accountable, and patient-focused.
