As the Director of Social Services (LSW), you will play a vital role in enhancing the quality of life for our residents by addressing their emotional, psychosocial, and discharge planning needs while supporting families throughout their healthcare journey.

Director of Social Services (LSW) Benefits & Schedule

• Full-time Day Shift

• Rotating weekend coverage (one weekend day)

• $5,000 Sign-On Bonus

• Current LSW or CSW required

• Long-term care experience preferred, but not required

• Prior supervisory or management experience preferred

• Current AHA BLS CPR Certification recommended

• Strong communication, organizational, and documentation skills

• Computer proficiency required

• Ability to work independently and as part of an interdisciplinary team

Director of Social Services (LSW/CSW) Responsibilities

• Lead and oversee the Social Services Department at Complete Care at Brick

• Develop and implement social service programs, policies, and goals

• Advocate for residents' psychosocial, emotional, and behavioral health needs

• Participate in interdisciplinary care plan meetings and collaborate with the clinical team

• Maintain accurate, timely, and compliant documentation

• Educate residents and families regarding Advance Directives and available resources

• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes

• Provide guidance and support to social services staff

• Assist residents and families with community resources and support services

• Perform additional duties as assigned by the Administrator
