As part of our hospice care team, you will assist patients with daily living activities while helping maintain comfort, dignity, and quality of life. Responsibilities Assist patients with personal care and hygiene Provide compassionate companionship and emotional support Support patient comfort and safety in the home setting Follow care plans directed by the RN Case Manager Document patient care accurately in EMR systems Communicate effectively with the hospice interdisciplinary team Qualifications Current California CHHA Certification required Valid driver’s license, auto insurance, and reliable transportation Ability to travel throughout assigned territories Hospice or home health experience preferred Bilingual English/Spanish is a plus, but not required Compassionate, dependable, and professional attitude #J-18808-Ljbffr
