Physical Therapist (PT) - Oasis at the Conch Republic

We are seeking a compassionate and skilled Physical Therapist (PT) to join our team at Oasis at the Conch Republic, a premier Assisted Living Facility in Key West, Florida. As a key member of our healthcare team, you will play a vital role in helping our residents achieve their maximum physical potential.

Responsibilities:

• Develop and implement individualized rehabilitation plans to address the unique needs of each resident
• Conduct thorough assessments, set goals, and track progress to ensure optimal outcomes
• Collaborate with other healthcare professionals to develop and implement comprehensive care plans
• Provide physical therapy interventions, including exercise programs, modalities, and manual therapy techniques
• Maintain accurate and detailed records of treatments and progress
• Educate residents, families, and caregivers on proper body mechanics, safety, and fall prevention techniques

Requirements:

• Current licensure as a Physical Therapist in the state of Florida

• Master's or Doctoral degree in Physical Therapy

• Experience working with older adults and/or individuals with chronic conditions
