This role partners with physicians and interdisciplinary teams to deliver high-quality, coordinated care; supporting organizational goals related to access, quality, and patient experience. What You’ll Do Clinical Care & Patient Services Conduct patient assessments, physical exams, and diagnostic evaluations Diagnose and manage acute and chronic conditions across the lifespan Develop and implement individualized treatment plans Prescribe medications and perform procedures within scope of practice Provide preventive care, screenings, and health maintenance services Patient Education & Care Coordination Educate patients and families on disease management, prevention, and wellness Collaborate with care teams to ensure coordinated, patient-centered care Support transitions of care and appropriate referrals within and outside the network Quality & Population Health Participate in quality improvement initiatives and care gap closure efforts Support population health strategies and value-based care programs Monitor patient outcomes and contribute to continuous improvement efforts Documentation & Compliance Maintain accurate and timely documentation within the Electronic Health Record (EHR) Ensure compliance with HIPAA, regulatory, and organizational standards Support coding, documentation accuracy, and billing requirements What You Bring Required Master’s or Doctoral degree in Nursing from an accredited program Board certification as a Family Nurse Practitioner (FNP) Prescriptive authority and DEA licensure (or ability to obtain) Strong clinical assessment, diagnostic, and decision‑making skills Excellent communication and collaboration skills Preferred Experience in primary care, community health, or FQHC settings Experience working with diverse or underserved populations Familiarity with EHR systems such as Epic Experience in team-based or integrated care environments Join Us If you are a mission-driven provider who is passionate about delivering high-quality care, improving access, and making a meaningful impact in the community, we encourage you to apply and be part of a team shaping the future of healthcare.
