Per Diem LPN - Hospice / Continuous Care

West Palm Beach, FL | Day & Night Shifts | Flexible Scheduling

Position: Licensed Practical Nurse (LPN)
Employment Type: Per Diem / As Needed
Primary Scheduling Need: Friday-Monday
Shifts: 12-hour Continuous Care - Day & Night shifts available
Scheduling: Flexible scheduling available
Travel Area: Up to a 50-mile radius of West Palm Beach, FL
EMR: HCHB (Homecare Homebase) experience required

Position Overview

We are seeking experienced Per Diem LPNs to provide compassionate, high-quality care to patients in the home and hospice setting.

This opportunity is ideal for an experienced LPN who is comfortable providing Continuous Care and working independently in the field. The primary need is Friday through Monday , with both 12-hour day and night shifts available . Flexible scheduling options may be available based on patient needs and staffing requirements.

Strong hospice experience and HCHB experience are required.

What You'll Do

• Provide high-quality, patient-centered nursing care in the home and hospice setting.

• Provide Continuous Care during assigned 12-hour shifts.

• Build meaningful, compassionate relationships with patients and their families.

• Provide nursing services according to the patient's individualized plan of care.

• Follow physician orders, agency policies, and applicable state and federal regulations.

• Observe patients for changes in condition and report adverse reactions to medications or treatments to the appropriate clinical team.

• Assist the physician and/or Registered Nurse with implementation of the patient's plan of care.

• Perform nursing procedures and specialized treatments within the LPN's scope of practice.

• Educate patients and families regarding medications, diet, treatments, and appropriate care.

• Accurately document patient care, interventions, and clinical observations in HCHB .

• Communicate effectively with the interdisciplinary care team.

• Maintain professional standards and compliance with agency policies and regulatory requirements.

Required Qualifications

• Current Florida LPN License in good standing.

• Current BLS certification .

• Minimum of 1 year of current, applicable nursing experience .

• Strong hospice experience required.

• HCHB/Homecare Homebase experience required.

• Graduate of an approved practical nursing program.

• Ability to travel within a 50-mile radius of West Palm Beach, FL .

• Ability to work independently in a home/hospice environment.

• Must be a U.S. Citizen .

Preferred Qualification

• Intravenous (IV) Certification.

All selected candidates must successfully complete the required credentialing and onboarding process, including:

• Influenza Vaccination 2026-2027 Seasonal

• Titers showing immunity - Hepatitis B, MMR, Varicella

• Mask Fit Testing

• Tdap (booster every 10yrs)

• Competency Testing

Orientation & Training

Training is mandatory before patient assignments can begin.

Step 1 - Online Learning

After successfully clearing the onboarding process, the LPN must complete the required online learning.

Step 2 - 8-Hour In-Person Training

After completing the online learning, the LPN must attend an 8-hour in-person training session , which will be scheduled as the worker's first day .

Training Location: Knowledge Center, West Palm Beach, FL 33407

Important: The 8-hour in-person training cannot be completed until all required online learning has been successfully completed.

Monthly Orientation Schedule

In-person orientation is scheduled once per month .

Candidates who do not complete the required online learning within the designated timeframe may be moved to the following month's orientation and training schedule .

Once onboarding, online learning, in-person training, and all required competency requirements have been completed, the LPN may begin receiving available patient assignments.
