Location: Monsey, NY
Position: Full time
Salary: $115,000-$120,000 per year

We are seeking a Registered Nurse to coordinate resident assessments and the MDS process. In this role, you will work closely with nursing staff and the interdisciplinary team to support accurate assessments, comprehensive care planning, and quality resident care.
Responsibilities

• Coordinate and complete resident assessments in accordance with applicable requirements and established timelines.
• Gather and review clinical information to ensure MDS documentation is accurate and complete.
• Collaborate with nursing staff and the interdisciplinary team to develop and update resident care plans.
• Monitor assessment schedules and help ensure documentation and submissions are completed on time.
• Communicate assessment findings and related care needs with appropriate team members.
• Provide guidance to staff on MDS processes and documentation.
• Maintain organized, accurate records and follow privacy and compliance standards.

Qualifications

• Current New York State Registered Nurse (RN) license required.
• MDS and care-planning experience in a long-term care setting.
• Strong organizational, communication, and documentation skills.
• Ability to coordinate effectively across clinical disciplines and meet deadlines.
