Responsabilidades Proporcionar apoyo social, navegación y defensa a pacientes de alto riesgo y bajos ingresos dentro de las comunidades para ayudarles a alcanzar sus metas de salud. Actuar como vecinos solidarios ayudando a los pacientes a abordar problemas sociales, médicos y comunitarios que contribuyen a la mala salud. Ayudar a los pacientes a enfrentar problemas de vida real que les impiden mantenerse saludables y a completar tareas de obtención de atención médica. Realizar actividades de salud proactivas como planificar comidas saludables y encontrar tiempo para hacer ejercicio. Requisitos Conocimiento de Google Suite y habilidad para entrada de datos. Excelentes habilidades de comunicación oral y escrita. Capacidad para seguir instrucciones escritas y orales y para recibir y abordar retroalimentación. Capacidad para ejercer juicio en la aplicación de servicios profesionales. Comodidad con el alcance telefónico, visitas a domicilio y trabajo en la comunidad. Experiencia previa como trabajador de alcance o título relacionado. Excelentes habilidades de gestión del tiempo. Bilingüe/multilingüe con capacidad para aprobar una prueba de fluidez en el idioma.
