Title: Occupational Therapist - Home Health Schedule:
- Shift: 8-hour days Assignment Details:
- Patient population: Adults / Geriatrics
- Typical caseload: Up to 6 patients per day Requirements:
- Licensure: Oregon and Washington licenses required
- Required certification: BCLS (BLS)
- Experience: Minimum 1 year skilled home health experience
- OASIS documentation experience required Roles & Responsibilities:
- Conduct home assessments and develop plans of care.
- Reassess patient progress and adjust treatment plans as necessary.
- Educate patients and families on safety and injury prevention.
- Assist patients with activities of daily living (ADLs). Compliance / Notes:
- Free parking available.
- COVID-19 vaccine not required.
