Referral Coordinator RN - Per Diem Day

Referral Coordinator (RN) – Providence Queen of the Valley Medical Center, Napa, CA. 8‑hour day shifts, Per Diem. Responsibilities Under the general supervision of the Acute Rehabilitation Nurse Manager and in collaboration with the treatment team and external entities, the Rehabilitation Admissions Nurse monitors, coordinates, and facilitates interdisciplinary plans of care, effective clinical resource management, discharge planning, and continuation of post‑acute care needs for assigned patients. The role promotes quality of care, cost‑effective patient outcomes, efficient resource utilization, and meets expectations of patients, families, referring physicians, agencies, and payer entities.
