The Licensed Practical Nurse (LPN) is responsible for providing and coordinating resident care services in accordance with applicable state nurse practice acts, assisted living/personal care regulations, memory care standards, physician orders, individualized service plans, and Company policies and procedures. The LPN supports the health, safety, dignity, and well-being of residents through clinical oversight, medication management, resident assessments, direct nursing care, staff support, and interdisciplinary collaboration. This position works collaboratively with residents, families, physicians, pharmacies, healthcare providers, and team members to promote quality outcomes and person-centered care while maintaining compliance with all applicable federal, state, and local laws, regulations, and licensing requirements. Essential Functions/ Duties: Essential duties may include, but are not limited to: Resident Care & Clinical Services Provide nursing care and support to residents consistent with individualized service plans, physician orders, scope of licensure, and applicable regulations. Assist residents with Activities of Daily Living (ADLs) including but not limited to bathing, dressing, grooming, toileting, ambulation, transfers, dining assistance, continence care, and personal hygiene. Monitor residents for changes in physical, emotional, behavioral, cognitive, and functional condition and promptly report concerns to the appropriate clinical leadership and/or physician. Perform routine resident rounds, wellness checks, and safety observations to ensure resident safety, comfort, supervision, and quality of care. Accurately assess, document, and communicate resident conditions, incidents, behaviors, injuries, and medical findings. Respond appropriately to resident emergencies, accidents, falls, elopement/wandering risks, behavioral escalations, medical emergencies, fire alarms, evacuations, and other urgent situations. Assist with resident admissions, assessments, transfers, discharges, service plan reviews, and ongoing care evaluations as assigned. Coordinate physician appointments, ancillary services, podiatry visits, therapy services, laboratory services, and follow-up care as needed. Administer treatments, injections, diabetic care, wound care, and other nursing procedures permitted within the scope of LPN licensure and applicable regulations. Assist with resident transportation and escort residents to meals, activities, appointments, and common areas as needed. Medication & Pharmacy Management Administer medications and treatments in accordance with physician orders, pharmacy guidance, applicable regulations, and Company policies and procedures. Provide oversight, guidance, and support related to medication administration activities in accordance with state law, facility licensure requirements, physician orders, and scope-of-practice limitations. Supervise and assist with medication cart management, narcotic counts, medication documentation, medication storage procedures, and medication inventory processes. Ensure Medication Administration Records (MARs/eMARs), physician orders, medication sheets, medication books, and medication inventories are accurate, current, and complete. Monitor residents for medication effectiveness, adverse reactions, side effects, medication refusals, and potential medication errors. Coordinate medication refills, pharmacy communications, medication changes, and medication order updates in a timely manner. Maintain compliance with controlled substance handling, storage, documentation, and sign-out requirements. Documentation, Compliance & Quality Assurance Complete timely, accurate, and thorough documentation in resident records, communication logs, incident reports, electronic health records, and related systems. Ensure compliance with HIPAA, resident confidentiality requirements, infection prevention standards, OSHA regulations, and applicable healthcare privacy laws. Demonstrate knowledge of infection prevention, exposure control, and emergency response procedures consistent with OSHA, CDC, and applicable state guidelines. Maintain familiarity with and adhere to all applicable federal, state, and local regulations governing personal care homes, assisted living communities, memory care communities, and residential care operations. Promote and protect resident rights, dignity, privacy, autonomy, and confidentiality in all aspects of care and service delivery. Participate in audits, surveys, inspections, mock surveys, regulatory preparedness activities, corrective action follow-up, and quality assurance initiatives as requested. Support leadership with policy implementation, process improvement activities, operational initiatives, staff education, competency support, and special projects as needed. Report suspected abuse, neglect, exploitation, unsafe conditions, injuries, incidents, and compliance concerns immediately in accordance with Company policy and mandatory reporting laws. Team Leadership & Collaboration Participate in shift reports, interdisciplinary meetings, care coordination discussions, and resident care planning activities. Provide clinical guidance, mentoring, and operational support to Medication Technicians, Resident Assistants, and other direct care staff. Assist with orientation, training, competency observations, and development of newly hired staff as assigned. Serve as the charge nurse or designated clinical lead in the absence of nursing leadership or other management personnel, as assigned. Communicate professionally and effectively with residents, families, physicians, pharmacies, healthcare providers, vendors, and team members. Housekeeping & Community Support Assist with maintaining a clean, safe, organized, and resident-centered environment. Support dining services, meal assistance, hydration monitoring, and resident engagement activities as needed. Answer phones, respond to call systems, and assist with general operational needs during assigned shifts. Participate in special projects, operational initiatives, community support functions, and temporary coverage assignments as directed by leadership. Perform other related duties and responsibilities as assigned. Other Duties: Perform other related duties as assigned. Supervisory Responsibilities: This position may provide clinical oversight, work direction, mentoring, and operational guidance to Medication Technicians, Resident Assistants, Caregivers, and other direct care staff; however, this position does not typically have formal hire, discipline, or termination authority unless specifically designated by leadership. Position Type/ Expected Hours of Work: Full-time, part-time, PRN, and variable shift positions may be available based on operational needs. Employees may be scheduled for day, evening, overnight, weekend, holiday, overtime, rotating shifts, temporary coverage assignments, or on-call responsibilities as necessary to meet resident care and staffing needs. Requirements Required Skills/ Abilities: Ability to safely administer medications and perform nursing procedures within scope of practice. Strong clinical assessment, observation, critical thinking, and documentation skills. Knowledge of nursing practices, medication administration, infection prevention, and resident care standards. Ability to recognize and appropriately respond to changes in resident condition and emergency situations. Ability to maintain professionalism, confidentiality, diplomacy, and sound judgment under all circumstances. Ability to communicate effectively verbally and in writing with residents, families, healthcare providers, and team members. Ability to establish and maintain effective working relationships in a team-oriented environment. Ability to read, write, speak, hear, and understand English sufficiently to safely perform essential job duties. Basic computer, electronic health record (EHR), and electronic medication documentation skills. Ability to prioritize tasks, manage multiple responsibilities, and adapt in a fast-paced healthcare environment. Ability to provide compassionate, resident-centered care while maintaining resident dignity, autonomy, and privacy. Education and Experience Requirements: Current and active Licensed Practical Nurse (LPN) license in the applicable state(s) of employment, in good standing. Graduate of an accredited practical nursing program. Current CPR and First Aid Certification (Required) High school diploma or equivalent (Required) Previous experience in assisted living, personal care, long-term care, dementia care, memory care, or senior living environments (Preferred) Experience with electronic medical records (EMR/EHR) systems (Preferred) Dementia care training or certification preferred. Experience providing guidance or support to direct care staff (Preferred) Work Authorization/ Clearances: Criminal History Clearances FBI fingerprint clearance (if required) Pre-employment drug screening (if applicable) Proof of eligibility to work in the United States Successful completion of all required orientation, competency evaluations, and ongoing education requirements Physical Requirements and Physical Demands: Ability to stand, walk, bend, stoop, kneel, lift, push, pull, and assist residents throughout scheduled shifts. Ability to lift, transfer, reposition, and assist residents utilizing proper body mechanics and approved safety equipment with or without reasonable accommodation. Ability to respond quickly and appropriately to emergencies and resident care needs. Ability to safely administer medications, treatments, and complete required documentation. Ability to operate medication carts, healthcare equipment, office equipment, and computers.
