Community Home Health Care is looking for a compassionate Overnight Nurse (RN or LPN) to provide quality care to patients in their homes during overnight hours. This role is ideal for a nurse who enjoys working independently while ensuring patients remain safe, comfortable, and supported overnight.

Responsibilities

Monitor and assess patient condition overnight
Provide skilled nursing care and support
Maintain accurate clinical documentation
Communicate with patients, families, and care team members

Requirements

Active RN or LPN license in Indiana
2+ years home health experience
Valid driver's license and reliable transportation
Strong communication and clinical assessment skills
