Registered Nurse, Pulmonary, Per Diem

Overview Position: Staff Nurse (Pulmonary - per diem) Department: Pulmonary Schedule: Part Time Boston Medical Center (BMC) is more than a hospital. It is the largest and busiest provider of trauma and emergency services in New England and emphasizes community-based care. BMC is the primary teaching affiliate of Boston University School of Medicine (BUSM) and a founding partner of Boston HealthNet. Position Summary The registered nurse adheres to the rules and regulations of the Massachusetts Board of Registration in Nursing. The nurse is primarily responsible for professional performance and direct patient care, including nursing assessment, diagnosis, planning, intervention, evaluation, and coordination of care for patients and their families across the health care continuum. The nurse is accountable for clinical, educational, quality and fiscal patient care outcomes using the model of care designed by the nursing department and in accordance with agency policies, procedures, protocols, guidelines and standards of practice. Position: Registered Nurse Department: Pulmonary Schedule: per diem Essential Responsibilities / Duties Standards of Care Consistently and effectively utilizes all steps of the nursing process in the provision of timely, safe, efficient and appropriate patient/centered care. Assesses the patient and family in a systematic and ongoing manner for both overt and covert signs and symptoms in accordance with the standards of clinical nursing practice. Collects pertinent data using appropriate assessment techniques. Prioritizes data according to the patient’s immediate condition or needs. Involves the patient, family/significant others and appropriate health care providers in the collection of data when appropriate. Uses clear, concise and effective written/oral communication and complies with departmental policies when sharing/documenting relevant patient care data. Identifies emergency/crisis situations, communicates promptly to the health care team, and responds appropriately when needed. Analyzes the assessment data to determine appropriate nursing diagnoses. Demonstrates critical thinking skills, derives and documents appropriate diagnoses from patient and family assessment data. Identifies expected outcomes individualized to the patient. Derives and documents realistic clinical and educational patient-centered outcomes consistent with diagnoses and standards of clinical nursing practice. Develops measurable outcomes with time estimates and provides direction for continuity of care. Mutually formulates outcomes with the patient, significant others and health care team when appropriate. Develops a plan of care that prescribes interventions to attain expected outcomes. Develops an individualized plan of care reflecting the patient’s condition and needs, with continuity of care. Ensures plan reflects current nursing practice standards, policies, procedures, protocols and guidelines. Plans using available resources and appropriate referrals to provide comprehensive care to patients and families. Implements interventions identified in the nursing and medical plan of care. Prioritizes patient care interventions based on fluctuating patient and unit needs. Provides care appropriate to the age groups of patients served; consults resources when patients outside age populations are present. Delegates appropriate patient care activities to co-workers and ensures completion in a timely manner. Maintains a safe, clean unit environment through risk management practices. Demonstrates safe technical competence (e.g., phlebotomy, IV insertion), informatics, and equipment monitoring/utilization. Implements patient care interventions and administers medications per standards, policies, and protocols. Educates patients and families regarding illness, procedures, treatments, plan of care, health maintenance and prevention. In emergencies, demonstrates effective decision making and coordinates a complete and coordinated response. Documents interventions according to departmental policy. Evaluates the patient’s progress toward clinical and educational outcomes. Conducts ongoing patient reassessments and evaluations. Evaluates effectiveness of interventions relative to desired outcomes, involving patient and appropriate providers as needed. Revises diagnoses, outcomes and care plans based on ongoing assessment data. Provides follow-up care in person via home visits and by telephone when needed and appropriate. Documents patient response and revisions in diagnoses, outcomes and plan of care per policy. (The above statements describe the general nature of work; it is not an exhaustive list of duties.) Education and Job Requirements Education Nursing degree: Diploma, ASN or BSN (preferred); ability to obtain BSN within 4 years Certificates, Licenses, Registrations Licensed to practice professional nursing as a registered nurse in the Commonwealth of Massachusetts. Current Basic Life Support (BLS) certification. Additional certifications may be required for certain practice areas. Experience 3-5 years experience preferred. Minimum of one year Ambulatory Care experience; pulmonary experience preferred. Telephone triage experience. Knowledge and Skills Basic computer proficiency; ability to access, enter, and interpret computerized data. Effective interpersonal skills to facilitate communication with the health care team, patients and families. Strong organizational skills; prioritization and efficient work habits. Multilingual skills preferred; relevant clinical experience in pulmonary or allergy preferred. Analytical ability to solve clinical nursing issues and participate in quality improvement. Ability to delegate appropriately and work as part of a team. Effective verbal and written communication appropriate to patient populations. Physical ability to meet core job responsibilities for the patient populations served. Ability to manage stress related to critical issues and organizational changes. This range reflects minimum qualifications and may vary with education, experience and certifications.
