• Conduct thorough patient evaluations in both inpatient and outpatient settings, crafting tailored treatment plans for each individual.
• Administer impactful physical therapy interventions concentrating on enhancing mobility, strength, balance, and overall functional abilities.
• Team up with nurses, physicians, case managers, and various rehabilitation experts to secure optimum patient outcomes.
• Educate patients and their families on the journey towards achieving therapeutic goals, providing insight into discharge planning and home exercises.
• Maintain meticulous documentation of evaluations, treatments, and progress in line with facility standards and regulations.
• Engage in interdisciplinary care team meetings and rounds to ensure seamless and coordinated patient care.

To thrive in this position, you will need:

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
• Current state licensure as a Rehabilitation Physical Therapist.
• CPR certification is preferred.
• Specialized certifications (for instance, in neurology or geriatrics) are advantageous but not mandatory.
