Optum Care at Home seeks a Behavioral Health Case Manager to deliver in‑home and telephonic support in Washington. You will assess, plan, coordinate, and monitor care for individuals with complex behavioral health and social needs. Responsibilities include care planning, coordinating with providers, and assisting with Medicaid LTSS and related benefits while documenting activities in electronic systems.
