Care Delivery Manager - Registered NurseWe are seeking an experienced and compassionate Care Delivery Manager - Registered Nurse to join our interdisciplinary team in Flushing, NY, supporting a high-quality PACE (Program of All-Inclusive Care for the Geriatrics) care model. In this leadership role, you will oversee daily clinical operations, ensure coordination of services, and support the delivery of culturally competent, person-centered care to our diverse geriatric population.Compensation: $55 - $64.50 Per HourSchedule: Monday-Friday 8:30 am-5:00 pmKey ResponsibilitiesOversee development and execution of individualized care plans that promote participant safety, satisfaction, and improved health outcomesLead and participate in Interdisciplinary Team (IDT) meetings, including Service Delivery Requests (SDRs), ensuring regulatory compliance and timely documentationProvide clinical leadership and represent Community Health Nurses (CHNs) within IDT discussionsReview and analyze Clinical Delivery reports to drive care coordination and operational decision-makingManage and allocate staffing resources to ensure appropriate coverage and seamless service deliveryServe as manager-on-call during designated off-hours to support urgent participant or caregiver needsMaintain strong communication across interdisciplinary teams and external partners to ensure care aligns with participant goals and preferencesQualificationsManaged Long-Term Care (MLTC) experience requiredStrong background in community health and/or home care, with solid management experience preferredActive, unrestricted New York State Registered Nurse (RN) license and registration requiredDegree from an NLN-accredited nursing school (RN required; BSN strongly preferred)Supervisory experience requiredMinimum of 2+ years of administrative experience in a management role within CHHA, LHCSA, MLTC, acute care, nursing home, or diagnostic and treatment clinic settings preferredExperience working with frail or elderly populations preferred (PACE training provided if needed)Case Management Certification recommended within 2 years of employmentSuccessful completion of NYSED-approved infection control coursework requiredMinimum of 8 continuing education contact hours, including mandatory training requirementsBilingual skills preferredStrong customer service, interpersonal communication, and cross-disciplinary collaboration skillsDemonstrated ability to manage complex care environments and operational prioritiesFlushing is one of New York City's most diverse and vibrant neighborhoods.
