Nurse (RN/LVN)

POSITION SUMMARY:

The Nurse provides nursing oversight and support for individuals receiving HCBS-AMH services, ensuring coordination of medical and behavioral health needs in accordance with HHSC requirements, the Uniform Managed Care Manual (UMCM), Medicaid policy, and agency policies and procedures.

Job Type: Full Time

Starting Pay: $61,300

Work Location: Houston Texas

RESPONSIBILITIES

· Conduct nursing assessments and ongoing health monitoring for participants.

· Monitor physical health conditions that may impact behavioral health functioning or service participation.

· Identify changes in medical or psychiatric status.

· Administer Medication to clients.

· Coordinate with prescribing providers, pharmacies, and hospitals as needed.

· Provide health education to clients and staff related to: Chronic disease management, Medication awareness, Infection control, Wellness and preventive care.

· Maintain accurate, timely, and compliant nursing documentation in accordance with: HHSC and Medicaid requirements.

PHYSICAL REQUIREMENTS

· Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds.

· Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day.

· Ability to walk with items in hand, grasp and reach as needed.

· Ability to perform with repetitive maneuvers and manual dexterity.

· Ability to work in an office environment.

Requirements:

QUALIFICATIONS

· Bachelor’s degree in public health

· RN or LVN License

· Experience working in the adult residential care industry is preferred.

· At least two (2) year of Mental Health experience preferred.

· Knowledge of psychiatric illness and treatment.

· Ability to work independently and be self-motivated.

· Excellent interpersonal skills with the ability to effectively work with individuals and groups.

· Highly organized and detail oriented, with the ability to work effectively in a fast-paced environment.

· Ability to flex work hours as needed.

· Willing to work some weekends and overnight shift.

· Possess strong interpersonal leadership, analytic, and problem-solving skills.

· CCL approved FBI and DOJ criminal background clearances.

· CPR/First Aid/BLS certification
