Senior Clinician, Pathways: Community Transition Specialist
Postgraduate Center for Mental Health
The Licensed Mental Health Professional will join a NY OMH-funded Adult Home based care transition team, serving individuals moving from Adult Home to the community. You will perform discharge planning, group and individual counseling, and develop daily living skills and coping strategies. Engage with community providers, conduct psychosocial assessments, coordinate housing supports, and participate in on-call coverage to support smooth transitions and ongoing community linkage. #J-18808-Ljbffr Postgraduate Center for Mental Health
