Care Coordinator Position at Clarvida - MaineCare Coordinator Location: Farmington, Maine (Home- and community-based) Employment Type: Full-time Salary: $52,000/yearAbout the RoleClarvida is seeking a Care Coordinator to provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with behavioral or mental health diagnoses.
