Registered Nurse (RN)

Position Summary

The Registered Nurse (RN) is responsible for providing comprehensive patient-centered care through clinical assessments, care coordination, patient education, and ongoing monitoring of patient conditions. This role works collaboratively with interdisciplinary healthcare teams and caregivers to ensure high-quality care and positive patient outcomes in the home health setting.

Responsibilities

Perform comprehensive evaluations of patients' physical, emotional, and overall health status
Develop, implement, and update individualized care plans based on patient needs and physician recommendations
Educate patients and caregivers on chronic disease management, medications, side effects, and self-care techniques
Coordinate care with physicians, therapists, social workers, and other healthcare professionals
Monitor patient progress and communicate changes in condition to the appropriate care team members
Maintain accurate and timely EMR documentation including assessments, interventions, and patient progress notes
Supervise Structured Family Caregivers, Home Care Aides, and other caregiving staff to ensure quality care delivery
Ensure compliance with care plans, agency policies, and healthcare regulations

Qualifications

Active Registered Nurse (RN) license in the applicable state
Previous home health or case management experience preferred
Strong clinical assessment and patient education skills
Knowledge of EMR documentation and care coordination processes
Excellent communication, organizational, and interpersonal skills
Ability to work independently and collaboratively within an interdisciplinary team

Education:

Bachelor's Degree - Nursing (BSN) (Desired)
Associate's Degree - Nursing (RN to BSN) (Required)
High School Diploma (Required)
