Substance Use Counselor

Summary To provide ongoing intensive case management to clients as defined by MHRH IHH Program Standards; outreach and crisis intervention to dually diagnosed (psychiatrically disabled/chemically dependent) clients, according to their level of care. This shall include general medication maintenance, substance abuse counseling, individual, family and group psychotherapy, independent living and vocational rehabilitation. Responsibilities Develop treatment plans in accordance with Gateway policies, procedures and Medicaid guidelines. Perform intakes within 30 days and ensure completion. Maintain medical record documentation in adherence to Gateway policies, procedures and IHH Medicaid guidelines. Educate staff regarding clinical intervention strategies related to symptomatology, substance abuse, sexual abuse, and family and relationship issues. Monitor medication to ensure client understands prescribed dosage, monitor side effects and compliance. Maintain caseload of clients assigned by Program Director/Supervisor. Coordinate activities, make appointments, and accompany client(s) as noted on Treatment Plan. Provide medical, psychiatric, substance abuse, individual, family, and group psychotherapy; assist with housing, entitlement, income maintenance, social, leisure activities, education and employment. Ensure clients’ active participation in treatment, maintenance of stable residence, development of ADL skills, social support network, and pursuit of employment. Coordinate treatment and groups for clients. Arrive on time for therapeutic, counseling and case‑management services and remain alert and responsive. Participate in daily staff organizational meetings and treatment planning review meetings. Provide ongoing assessment of client’s mental illness symptoms and response to treatment. Offer problem‑solving, skill training, supervision and environmental adaptations to assist clients with activities of daily living. Provide case‑management referral to appropriate services. Assess clients in crisis who are at risk of harming self or others per procedure. Screen for substance abuse and orient clients to IHH treatment options. Attend agency or supervisor required in‑services, other agency in‑services, and at least one course, seminar or conference per year. Participate in direct contact with clients, families and collateral contacts according to treatment goals. Work minimum 22 hours per week, primarily focused on substance‑abuse interactions. Perform other duties and responsibilities as assigned or needed. Qualifications Bachelor’s or Master’s Degree in a behavioral health related field. CADC or LCDP licensure required by 9/30/2025 or equivalent masters‑level behavioral health license (LCSW, LICSW, LMHC-A, LMHC, LMFT-A, or LMFT) required. CPR training within 90 days of employment and maintain annually. Valid RI license at all times and use of car or other transportation during normal working hours. Annual physical examination and Mantoux test. Maintain automobile liability insurance coverage for vehicle used in accordance with center requirements. Rhode Island Certification as a Mental Health Case Manager within one year of hire date. Rhode Island Certification as a Chemical Dependency Professional within two years of hire date. Continued education required to maintain certifications. Knowledge of community resources and understanding of mental health and chemical dependency issues. Strong interpersonal skills to cooperate at all levels within the center. Oral and written communication skills including clear speech, grammar, spelling and legibility. Basic mathematics skills: addition, subtraction, multiplication, division of whole numbers, common fractions and decimals. Job Knowledge, Skill, and Ability Must understand community resources, mental health and chemical dependency issues, and possess the interpersonal, communication, and basic math skills necessary for effective client interaction and record keeping. Ethics and Professional Conduct Use client information only as necessary for job duties and in accordance with Gateway Healthcare Inc. policies and legal requirements. Comply with agency policies, procedures, and codes of conduct in all client, co‑worker, and community interactions. Physical/Environmental Demands The job requires frequent use of hands to handle objects, office equipment, and controls, speaking and hearing. It frequently requires walking, sitting, reaching with hands and arms, climbing, standing, stooping, kneeling, crouching, and occasional lifting up to 10 pounds. Site may involve moderate noise levels and occasional outdoor weather conditions.
