Performs care coordination for patients i.e. abnormal results, triage, RN verification of vaccinations and other injections, management of in-baskets. (prior authorizations and forms) Oversees clinical care essential to the assessment, promotion, maintenance and restoration of patients’ health and wellness. Minimizes risk of harm to patient, their family/designated care partner(s) by following established nursing practices and protocols Uses information technology to communicate, manage knowledge, and mitigate errors Uses data to monitor the outcomes of care processes and support ethical decision making; uses improvement methods to design and test changes to continuously improve the quality and safety of patient care Functions effectively within primary care inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
