Physician Assistant

Signify Health seeks a mission-driven Physician Assistant to perform in-home and virtual clinical evaluations for diverse patient populations. You’ll conduct comprehensive health assessments, manage chronic and acute conditions, and document findings in EHR systems to support value-based care and risk adjustment. Collaborating with interdisciplinary teams, you’ll coordinate follow-up care, close care gaps, and leverage modern technology and data insights to improve outcomes while maintaining a flexible, patient-centered work–life balance in a hybrid environment.
