University Home Care Inc is seeking a compassionate and skilled Home Visits Registered Nurse to provide exceptional in-home nursing care to patients in Wayne County, MI. This role involves delivering personalized health services, assessing patient needs, and promoting wellness in a comfortable and supportive home environment. The ideal candidate will have strong clinical knowledge, excellent communication skills, and a commitment to improving patient outcomes through attentive and individualized care. Responsibilities

Conduct in-home patient assessments and develop individualized care plans.
Provide skilled nursing services such as wound care, medication management, and chronic disease monitoring.
Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated patient care.
Educate patients and their families on managing health conditions and promoting wellness.
Document all patient interactions, treatments, and progress accurately and timely.
Maintain compliance with all state and federal regulations governing home healthcare.

Requirements

Valid Registered Nurse (RN) license in the state of Michigan.
Minimum of 1 years nursing experience, preferably in home health.
Kinnser Experinces is a plus
Strong assessment, clinical, and critical thinking skills.
Excellent interpersonal and communication abilities.
Reliable transportation and ability to travel to patient homes in Wayne County, MI.
Current CPR and Basic Life Support (BLS) certification.
