JOB STATUS: Full-Time, Exempt

SCHEDULE: Monday- Friday, 8am-4pm

Summary:

Provide leadership, supervision, coordination, and oversight of Habilitation programs and services for adults with disabilities. This position is responsible for planning, implementing, monitoring, and evaluating habilitation activities and individualized services to promote personal growth, independence, socialization, community integration, choice-making, and self-direction. The Habilitation Manager ensures services are delivered in accordance with individual needs, established program standards, applicable regulations, quality requirements, and agency policies and procedures.

Key Responsibilities:

• Plan, supervise, coordinate, and evaluate all Habilitation program activities and staff
• Create an environment that promotes participation, socialization, independence, personal growth, and involvement in functional, age-appropriate activities
• Assess and monitor the personal, social, vocational, and community integration needs of each person served
• Coordinate and maintain a Person-Centered Individual Support Plan (PCISP) for each person served
• Ensure services are delivered according to individualized assessments, service plans, program guidelines, and established goals and objectives
• Monitor documentation of activities and services designed to meet the individual needs of persons served
• Establish and monitor procedures for documenting and evaluating individual progress toward goals and objectives
• Conduct or oversee quarterly written reassessments of each person’s progress and modify individual service plans and methods of service delivery as needed
• Develop and implement functional, age-appropriate activities using current best practices and systematic instructional methodologies
• Ensure activities address measurable, time-referenced goals and objectives and support the generalization of skills across settings, people, and time
• Utilize community resources, volunteers, social groups, and other natural supports to expand opportunities for persons served
• Promote personal autonomy, self-direction, and informed choice by maximizing opportunities for individuals to make decisions regarding their activities and services
• Monitor monthly activity calendars for applicable staff and programs
• Provide staff training based on the needs of direct service personnel and the requirements of individual service plans
• Conduct routine staff meetings to support planning, communication, organization, and effective service delivery
• Participate in ongoing program evaluation and quality assurance activities
• Provide summary documentation of service outcomes for monthly reporting and program evaluation
• Participate in staff development and professional training designed to maintain and improve job knowledge and performance
• Maintain communication and serve as a liaison between Blue River Services and state agencies, peer agencies, families, advocates, guardians, and legal representatives involved in service coordination
• Develop and maintain natural community support systems appropriate to each person’s needs, interests, abilities, and lifestyle
• Coordinate volunteer involvement and community resources when appropriate
• Advocate for each person served and support individuals in understanding and exercising their rights and responsibilities as citizens
• Assist individuals in researching and accessing community organizations, civic or political groups, religious organizations, and other activities of their choice
• Provide habilitation instruction, supervision, and counseling to persons served individually and in groups as needed
• Recommend employment opportunities, job placement, ancillary services, and other activities based on the needs and interests of persons served
• Coordinate and monitor vocational and habilitation information for supervised group living residents as applicable
• Annually update Habilitation programs, curricula, activities, and instructional materials
• Maintain security and confidentiality of person served records and employee information
• Maintain knowledge of current issues, trends, practices, and best practices related to services for adults with disabilities
• Attend required committee meetings, trainings, conferences, and other agency activities
• Attend out-of-town training sessions, including those requiring overnight stays, as necessary
• Complete all other duties as assigned by the supervisor

Qualifications & Skills:
• Strong leadership, supervisory, organizational, and decision-making skills
• Ability to supervise, direct, train, and evaluate Habilitation staff
• Ability to develop, implement, monitor, and revise individualized service plans
• Knowledge of person-centered planning and individualized support practices
• Ability to promote independence, self-direction, choice-making, socialization, and community integration
• Strong written and verbal communication skills
• Ability to maintain accurate records, documentation, reports, and service information
• Ability to establish and maintain professional relationships with persons served, families, guardians, advocates, coworkers, state agencies, community partners, and the general public
• Ability to encourage, reinforce, and provide constructive support to persons served and staff
• Ability to use word processing, database, spreadsheet, and other computer software
• Ability to coordinate multiple programs, staff, activities, and service requirements
• Ability to maintain confidentiality of employee and person served information
• Ability to interpret and implement agency policies, procedures, service requirements, and regulatory standards
• Ability to advocate for the rights, dignity, choice, safety, and independence of persons served
• Ability to travel to trainings, meetings, service locations, and other required activities
• Ability to work effectively with adults with disabilities and respond appropriately to individual needs

Physical & Work Requirements:

• Ability to lift and carry up to 50 pounds for a distance of approximately 15–20 feet
• Ability to stoop, bend, and assist persons served as necessary
• Ability to perform physical activities associated with supporting and assisting persons served
• Ability to operate a motor vehicle for business-related travel and maintain dependable transportation for trainings, meetings, client transportation, and other position-related activities
• Local and occasional out-of-town travel required
• Overnight travel may be required for training and professional development
• Ability to maintain a safe working environment and follow applicable health and safety procedures
• Ability to work in a variety of community and program settings as required

• Maintain a valid operator’s license, including For-Hire endorsement when applicable

• Maintain a driving record check with no more than two points and no incidents of hazardous or reckless driving • Maintain current proof of vehicle liability insurance

Certifications & Training Requirements:

• Maintain current First Aid certification
• Maintain current CPR certification
• Complete and maintain Preventing Disease Transmission (PDT) training
• Complete and maintain Certified Restraint Training (CRT)
• Complete Medication Administration training, including Core A and B
• Complete Home and Community Support Professional (HCSP) training and maintain registry requirements
• Complete Person Served Specific Training
• Complete all required initial and annual training, including Incident Reporting Procedures, Person-Centered Planning/Individual Rights, Abuse, Neglect and Exploitation, Health and Wellness, Communication, Medication Administration, Emergency/Evacuation Procedures, Personnel Policies, Safety During Lifting/Transferring, PCISP, and other applicable training
• Maintain all training and certifications required by the agency, program, and applicable regulatory or funding authorities

Requirements

Education/Experience Requirements:

• High school diploma from an accredited school or equivalent required

• Minimum of two years of experience working with individuals with disabilities

• Experience working with the aging population

• Bachelor’s degree in education, sociology, or a related field preferred

• One year of supervisory experience preferred
